Charge Conference Forms
The 2011 Charge Conference Forms are now available for download in the following formats:
Each form is available in PDF format for users who wish to print the forms and fill them out by hand, or who have PDF editing software.
Microsoft Office users may also download the forms in Word or Excel format so that they can be completed on computer.
Be certain to download the Instruction Sheet and Charge Conference Forms list to assure that you have a complete packet.
To download each form, right click on the word "Download" in the appropriate column and select "Save Target as..."
*All Microsoft Downloads are in Word except for Form #3 - BCBS Rate Sheet and Form D - Pastor Support Worksheet which are in Excel
**All forms must be completed and submitted on the day of your Consultation/Pre-Charge Conference Review. Contact the District Office if you are unsure of the date and time of your appointment.
