Year End Statistical Reports
Deadline for submission: January 15, 2010
The 2009 Year End Statistical Report Forms are now available for download in PDF and MSWord and allow for electronic completion.
Please note that the Table I, II, and III reports in PDF format cannot be saved after completion. You will need to print a copy before closing
the file. If you wish to use PDF and be able to save it electronically, the District Office can scan your printed form.
To download each form, right click on the word "Download" in the appropriate column and select "Save Target as..."
